Expenses Claim Form	Anyone	Standard Deanery expenses claim form For all expenses not covered by the following:
Please complete all sections of the form and sign the 'Declaration by Claimant' section before sending the form to the General Practice Office. Failure to do this will result in return of the form and delay to the payment.
Staff Travel & other Expenses P8	All Staff employed by the Deanery at the GP Office	Monthly travel and subsistence costs

Please ensure that all claims are submitted before the end of the financial year in which cost was incurred (financial year 1st April to 31st March).
