PAPERWORK FOR GP SHOS

This page provides access to all the paperwork GP SHOs should need for recording and certifying their training in hospital and community (non-GP) posts of any length.

  • You should download these blank forms, as many as you need, print them out and keep them in a folder as a personal record of training.
  • You should take these to your supervisor for completion as required - your supervisor will not have copies!
  • It may be an idea to check that your supervisors have a copy of the Guide and provide one if necessary!

Note: - the 2 downloads should be identical - choose the red one first and the orange one if the red doesn't work!
The red download is from the JCPTGP website and should be bang up-to-date (but may be unreliable).
The orange download is from the Deanery website and is current at July 2005.

 

The SHO Report (JCPTGP)

You will need one of these for each attachment. It includes:

  • Recording sheets for initial, middle and end reviews.
  • Completion Forms (VTR/2 & RITA) C, D & E
  • Referral Form (for use if not satisfactory (Forms D or E))
  • Rating scales for use at mid-point and end-point
 

The Guide (JCPTGP)

The Guide (Deanery)

You will need one of these for the whole of your training. It includes:

  • Full instructions on the use of the report.
  • Generic attributes for GPs
  • Attributes specific to various specialties
  • The forms for 360 degree review.
