The Structured Trainer's ReportThis report will be completed
by your final GP trainer and covers the aspects of your performance
which are difficult to assess by any means other than direct observation.
You are also required to submit a Out of Hours Certificate of Completion
with your Structured Trainer's Report which is downloadable from the
'Out of Hours' page. The OOH workbook is required to be kept by
the registrar and the practice should keep a copy. And attitudes - teamworking, acceptance of responsibility etc You can download a copy from the National Office for Summative Assessment's website. The reports are no longer obtainable from your Deanery office.
